* Measurement Year 2022 technical specifications referenced for purposes of this initiative.

1. Colorectal Cancer Screening (COL): The percentage of members 50–75 years of age who had appropriate screening for colorectal cancer.
2. Controlling High Blood Pressure (CBP): The percentage of members 18–85 years of age who had a diagnosis of hypertension (HTN) and whose blood pressure (BP) was adequately controlled (<140 /90 mm Hg) during the measurement year.
3. Hemoglobin A1c Control for Patients With Diabetes (HBD): The percentage of members 18–75 years of age with diabetes (types 1 and 2) whose hemoglobin A1c (HbA1c) was at the following levels during the measurement year:
   • HbA1c Control (<8.0%).
   • HbA1c Poor Control (>9.0%)

* For ease of review, this dashboard has separated quantitative results for this measure between the two indicators: “HBD Control” and “HBD Poor Control”.

4. Prenatal and Postpartum Care (PPC): The percentage of deliveries of live births on or between October 8 of the year prior to the measurement year and October 7 of the measurement year. For these members, the measure assesses the following facets of prenatal and postpartum care.
   • Timeliness of Prenatal Care. The percentage of deliveries that received a prenatal care visit in the first trimester, on or before the enrollment start date or within 42 days of enrollment in the organization.
   • Postpartum Care. The percentage of deliveries that had a postpartum visit on or between 7 and 84 days after delivery.
   • For ease of review, this dashboard has separated quantitative results for this measure between the two indicators: “PPC Prenatal” and “PPC Postpartum”.
5. Child and Adolescent Well-Care Visits (WCV): The percentage of members 3–21 years of age who had at least one comprehensive well-care visit with a PCP or an OB/GYN practitioner during the measurement year.

HEDIS measures are specified to use one or more data collection methods. For this dashboard, participating plans could report using either the Administrative or Hybrid methods.

• Administrative: Data gathered from claims, encounter, enrollment and provider systems.
• Hybrid: Data gathered from administrative and medical record data.

Data derived purely from administrative sources reflect rates that consider every eligible member and occurrence of the event calculated by a measure. All other data are based on members and services drawn in a systematic sample specified by NCQA.

The dashboard default view is “All”, combining collection methods to show all participating plan data, as this is most helpful when considering race and ethnicity data completeness. However, evaluating performance rates separately by collection method may be more appropriate when, for example, comparing to external benchmarks or sources.

For purposes of this dashboard and report, two types of race and ethnicity data sources are described, aligned with HEDIS MY 2022 reporting requirements:

• Direct Data: Data collected directly from members, reflecting members’ self-identification.
• Indirect Data: Race and ethnicity values assigned using secondary or alternate data sources, not member self-report. Some commonly used indirect methods combine geographic data with additional imputation methods such as surname analysis.
